What you actually will be doing
An important distinction: MRIguidance is a MedTech company that sells to health systems, not a health system itself. This means your reimbursement work is not about managing contracts you already have — it is about building the conditions that make reimbursement possible for your customers, so they can adopt and sustain BoneMRI.
In the USA, BoneMRI has been assigned HCPCS code G0566, and reimbursement operates differently depending on the care setting:
- Hospital outpatient departments bill the technical component via G0566, reimbursed through OPPS under APC group 5721
- Non-facility settings (physician offices, imaging centers) may bill globally or split technical and professional components; rates are carrier-priced by the local MAC
- Physicians bill the professional component (interpretation) using G0566 with modifier -26; because G0566 carries no assigned RVUs, crosswalk codes are used to benchmark payment
Future activities will include relationship building and negotiations with Medicare, Medicaid, private payors, and hospital administrations, as well as collaborative work with revenue cycle managers to optimize the chargemaster for incorporating BoneMRI to include the additional scantime/resources and gathering of financial evidence to apply for Cat III or Cat I CPT codes.
Between 5 and 10 US sites are currently establishing ordering and billing workflows, supported remotely from our team in the Netherlands. We are in an active scale-up phase: driving first charges, managing early denials, building MAC relationships, and working toward stable and predictable reimbursement across all three settings. This is the classic early-stage reimbursement ramp — and navigating it well is the core challenge of this role.
In Europe, equivalent reimbursement pathways are at various stages across target markets. You will contribute to both geographies.
Like any scale-up, we all wear multiple hats. In the first year, you will also support the project management of new customer launches: coordinating go-lives, aligning clinical and billing stakeholders, and ensuring new sites reach their first successful claim. After this initial period, your primary focus will be on reimbursement strategy and execution.
You will report directly to Peter Seevinck (Founder & CSO) and Guido Zijl (CEO), your key partners in defining strategy and navigating payer relationships globally.
Your responsibilities
USA Billing Ramp & Payer Relations
- Support active hospital and imaging center sites in building and validating ordering and billing workflows for G0566 across all care settings
- Drive charge capture at Medicare and commercial payers; monitor claim submission and reimbursement rates per setting
- Proactively manage denials and appeals, identifying patterns across sites and refining guidance accordingly
- Advise sites on ICD-10 code selection for BoneMRI claims (leveraging the diagnosis codes that supported the original MRI) and on prior authorization workflows as the code matures
- Build and maintain relationships with MACs, Medicare/Medicaid, and key commercial payers
- Support crosswalk code strategy to benchmark carrier-priced physician component payments
- Work toward establishing stable, predictable reimbursement across hospital outpatient, non-facility, and physician settings
- Evaluate and advise on the role of Revenue Cycle Management (RCM) outsourcing partners, assessing where third-party support for prior auth, submissions, denials, and appeals adds strategic and operational value
Market Access & Reimbursement Strategy
- Define how BoneMRI will be reimbursed across target countries and jurisdictions
- Assess whether expansion to additional code types or reimbursement pathways is warranted as the product and evidence base mature
- Build country-by-country launch strategies in close coordination with the commercial team
- Advise on pricing to ensure sustainable reimbursement outcomes
HCP Education & Billing Support
- Educate hospital and imaging center billing teams on G0566 coding, setting-specific billing rules, ICD-10 selection, and prior authorization workflows
- Guide sites through the full claims process: from correct code and modifier application to successful payment
- Develop and maintain clear billing guidance materials that sites can operationalise within their own EHR and RCM systems
- Educate customers on budget impact models and the economic case for BoneMRI
New Customer Launch Management
- Project-manage go-lives for new hospital and imaging center customers, coordinating clinical, billing, and IT stakeholders
- Develop structured onboarding strategies to drive adoption and first successful claims
- Monitor post-launch reimbursement performance and resolve operational blockers
Qualifications
Minimum
- Bachelor’s or Master’s in Health Economics, Life Sciences, Healthcare Management, or a related field
- 3–5+ years of experience in reimbursement, market access, or health economics — with hands-on exposure to the USA payer landscape (Medicare, Medicaid, commercial payers)
- Working knowledge of HCPCS/CPT coding, OPPS, APC payment logic, and Medicare billing across multiple care settings (hospital outpatient, non-facility, physician office)
- Familiarity with MAC processes, carrier-priced codes, crosswalk methodology, and the Medicare appeals and denial cycle
- Experience supporting or working with health systems to establish or improve billing workflows — whether from inside a health system or from the vendor side
- Ability to distinguish technical and professional reimbursement components and advise on setting-specific billing strategy
- Strong analytical and communication skills: able to translate payer dynamics and claims data into clear guidance for billing teams and strategic recommendations for leadership
- Comfortable working with a high degree of autonomy and across time zones (Utrecht – USA)
- Excellent English; additional European languages are a strong plus
- Willingness to relocate to the Netherlands, at least for the first 1-2 years.
- Able to travel to the USA on a regular basis.
Preferred
- Experience at or with a MedTech vendor or SaMD company — building reimbursement from the vendor side for a newly coded product
- Experience managing the early billing ramp for a new HCPCS or CPT code: driving first charges, managing early denials, and building payer acceptance
- Experience evaluating or managing RCM outsourcing partners
- Knowledge of EU payer systems, HTA bodies, or country-level reimbursement submissions
- Existing network with MACs, Medicare, or major commercial payers
- Dutch, German, or French language skills
- Eligible for a Dutch work visa (or EU/EEA citizen)
